About the role
The Community Health Worker provides proactive, individualized coaching and resource linking to a group of patients via telephone and home visits, with an end goal of supporting healthy living and behavioral change.
Hours: 8 hours per day, 5 days per week.
What you'll do
- Provide individualized health coaching to patients by phone and home visit
- Link patients to community resources
- Collaborate with multidisciplinary team members to provide the best care
- Support patient healthy living and behavioral change
What you bring
- High school diploma or equivalent; bachelor's degree preferred
- 2 years of health coaching, wellness, health promotion or outreach experience
- Community Health Worker Certificate or ability to obtain within one year of hire
